Starting Your Allotment

You will need Adobe Acrobat installed on your computer to print or view these forms.

For instructions on how to start your allotment on-line click Mypay instructions. You will need your PIN number.

Below you will find a link which will take you to an allotment authorization form.  This authorizes your military finance office to send your monthly payment directly to Security National via First Citizens Bank.

Please print a copy of the authorization form and return the completed copy to Security National.

Before returning your form be sure the following areas have been completed:
1. Your Social Security Number
2. Your Name, Address, and Phone Number
3. Your Signature and Date
4. Your Date of Birth

Return the completed Payment Authorization form via fax to:  (513) 459-6497.

Along with your authorization form you will also need to submit a  DD2558 Military Allotment Form which also can be obtained at your finance office. 

You need the following information to complete the DD2558 Military Allotment Form:
1. The Allottee is First Citizens Bank.
2. The Allottee’s address is: PO Box. 988 Radcliff, KY. 40159
3. The Allottee’s routing number is 083901825.
4. The account number is the Customer’s Social Security Number.
5. The account type (for First Citizens Bank) is a savings account.
